LPN, Behavioral Health Support: 60-72 hrs/pay period (Days)
POSITION/JOB SUMMARY
The Behavioral Health Support Specialist – LPN provides nursing, behavioral health support, scheduling, care coordination, and patient outreach in an outpatient behavioral health setting. The position works closely with behavioral health and medical providers to promote patient access, safety, and continuity of care.
This position is scheduled varied day shifts Monday - Friday from 8:00 a.m.-5:00 p.m. Applicant can choose to be regularly scheduled at 60 or 72 hours per two-week pay period.
ESSENTIAL FUNCTIONS
- Applies JMHS standards and applicable regulatory requirements, including Minnesota Department of Health, OSHA, and CDC guidelines, in the delivery of behavioral health services.
- Demonstrates knowledge of mental health conditions, behavioral symptoms, and therapeutic interventions appropriate to the LPN scope of practice.
- Utilizes de-escalation techniques and trauma-informed care principles to promote a safe and supportive environment for patients and staff.
- Administers medications as ordered, monitors for side effects and patient response, and supports medication adherence within the LPN scope of practice.
- Obtains vital signs, completes medication reconciliation, and assists with nursing assessments and other clinical duties as appropriate.
- Monitors patient status and promptly communicates changes in behavior, mood, symptoms, or condition to the appropriate provider or therapist.
- Accurately documents patient encounters, observations, interventions, and care coordination activities in the electronic health record (EHR).
- Manages and coordinates behavioral health referrals from initiation through completion, including scheduling, obtaining necessary records, communicating with patients and outside providers, and following up on referral status.
- Coordinates care and assists with referrals to psychiatry, counseling, primary care, and community-based services.
- Participates in care team huddles and case reviews to support continuity of care and integration of behavioral and physical health services.
- Supports behavioral health crisis response, including gathering clinical information, coordinating with providers and emergency services, and assisting with safety planning as directed.
- Assists with patient education, group activities, or other therapeutic programming as directed by licensed providers.
- Maintains familiarity with the proper use and maintenance of clinical equipment relevant to the position.
- Protects patient dignity, privacy, confidentiality, and emotional safety throughout all interactions.
- Maintains appropriate professional boundaries and demonstrates ethical, respectful, and compassionate conduct in all patient interactions.

QUALIFICATIONS
- Graduate of an accredited LPN program (required)
- Current Minnesota LPN license in good standing (required)
- Experience in behavioral health or a related care setting preferred
- Training in crisis intervention or mental health first aid preferred (or willingness to obtain)
- Skilled in basic clinical procedures
- Demonstrates empathy, patience, and adaptability in high-stress situations
- Strong organizational and communication skills • Current BLS/CPR certification (required)
