POSITION/JOB SUMMARY:

The Registered Nurse (RN) is responsible for providing and coordinating safe, competent, and compassionate nursing care to residents in a skilled nursing facility in accordance with the Minnesota Nursing Practice Act, federal and state regulations, and facility policies and procedures. The RN ensures the delivery of individualized, comprehensive, and resident-centered care that supports dignity, continuity, quality of life, and optimal clinical outcomes.

The RN assesses resident conditions, identifies nursing needs, participates in the development and implementation of care plans, evaluates resident outcomes, and ensures complete, accurate, and timely clinical documentation. The RN appropriately delegates nursing tasks to LPNs, CNAs, and TMAs based on resident needs, staff competency, and applicable scope of practice, while remaining accountable for nursing care provided.

This role requires strong leadership, sound clinical judgment, effective communication, and excellent customer service skills to build and maintain positive working relationships with residents, families, coworkers, and members of the interdisciplinary team. The RN may function as a primary nurse or charge nurse and provides supervision, guidance, and support to other nursing personnel.

Shift Differentials:

Night: $3.50/hr
Weekend: $2.50/hr

ESSENTIAL FUNCTIONS

  • Utilize the nursing process (assessment, planning, implementation, and evaluation) to provide safe, competent, and compassionate resident care.
  • Conduct comprehensive assessments of residents’ health status and care needs and ensure accurate, timely documentation.
  • Observe, monitor, document, and report changes in residents’ conditions to the physician, supervisor, family/responsible party, or other appropriate individuals.
  • Initiate, obtain, transcribe, implement, and discontinue physician or practitioner orders as appropriate.
  • Administer medications, treatments, and nursing procedures in accordance with physician orders, care plans, facility policies, and regulatory standards.
  • Review laboratory results and notify the physician or provider when indicated.
  • Monitor new admissions, transfers, and readmissions and initiate appropriate nursing care and documentation.
  • Obtain specimens and perform clinical procedures as needed.
  • Monitor and document vital signs, weights, oxygen saturation, intake/output, and other clinical indicators as directed or indicated by resident condition.
  • Recognize and respond appropriately to significant changes in resident condition and communicate with appropriate clinical and leadership personnel.
  • Support residents’ physical, emotional, spiritual, and psychosocial needs, including providing compassionate end-of-life care and supporting families.
  • Assist residents with personal care needs as necessary to support safe, quality care.
  • Conduct rounds as indicated by residents’ needs and respond promptly to call lights.
  • Supervise, direct, and coordinate the work of LPNs, CNAs, TMAs, and other support staff to ensure safe and effective resident care.
  • Provide leadership, guidance, and a professional example to nursing staff during assigned shifts.
  • Receive reports from nursing assistants and other staff and follow up on concerns or changes in resident condition.
  • Provide accurate and timely shift reports and ensure clear hand-off communication to support continuity of care.
  • Serve as a liaison between residents, families, physicians, leadership, and interdisciplinary departments.
  • Communicate pertinent resident information to appropriate departments such as dietary, housekeeping, maintenance, and administration.
  • Maintain medication carts, medication rooms, and nursing work areas in a clean, organized, and regulatory-compliant manner.
  • Perform narcotic counts and the controlled substance reconciliation at the beginning and end of shifts.
  • Complete required documentation accurately and timely, including daily charting, exception charting, behavior documentation, and clinical notes.
  • Complete required documentation related to admissions, transfers, discharges, and deaths.
  • Investigate resident incidents or accidents and complete required variance or incident reports according to facility policy.
  • Report resident injuries or accidents to the Director of Nursing (or designee), Licensed Nursing Home Administrator, provider, and responsible party according to facility risk management protocols.
  • Follow infection prevention and control practices and facility infection control policies.
  • Maintain a clean, safe work environment by identifying, documenting, and reporting hazardous conditions, equipment malfunctions, or safety concerns.
  • Use proper body mechanics and ensure safe use of equipment such as lifts, transfer belts, and other assistive devices.
  • Activate maintenance or other support services when environmental or equipment issues arise.
  • Complete required paperwork, clerical duties, communication logs, and unit documentation as assigned.
  • Participate in quality assurance, quality improvement, and risk management activities that promote resident safety and quality of care.
  • Review communication tools such as daybooks, communication logs, and posted updates to remain informed of residents and facility changes.
  • Attend required meetings, workshops, in-services, and continuing education programs and maintain required job competencies.
  • Maintain punctuality, attendance, and scheduling expectations in accordance with facility policies.
  • Utilize available resources and reference materials to support safe and effective nursing practice.
  • Provide constructive input regarding resident care, supplies, equipment, or operational improvements.
  • Support teamwork by assisting colleagues and promoting a cooperative work environment.
  • Assist with staffing coordination and operational needs as directed.
  • Perform other duties as assigned by the supervisor.

QUALIFICATIONS

  • Associate’s degree (U.S. or foreign equivalent) in nursing or a closely related field. Must possess at least one of the following: full and unrestricted license to practice professional nursing in the State of Minnesota, evidence of NCLEX-RN exam pass, OR CGFNS (aka TruMerit) Certification.
