MDS Coordinator: Care Center $68,369.00-$95,264.00 yearly
POSITION/JOB SUMMARY:
The MDS Coordinator is part of the interdisciplinary professional senior living care team for the care center which collaborates with the long-term clinical and billing team to ensure proper codes are recorded in the medical record that is reflected in the billing process. The MDS Coordinator is responsible for up-to-date assessments of the residents and oversees the interdisciplinary assessment for all care center residents are completed by the interdisciplinary team within the scheduled timeframe. The MDS Coordinator monitors resident care by assessing procedures, documentation, speaking to residents/family members, and staff to ensure proper medical codes are captured for billing and Case Mix scoring.
This FLSA Exempt Full-time position typically works Monday-Friday 8:00am-4:30pm and reports to the Director of Senior Living.
ESSENTIAL FUNCTIONS
- Manage and assess resident care to ensure the facility provides consistent quality care.
- Participates in the Interdisciplinary Care Center Team (IDT).
- Assess and monitor resident care and wellbeing.
- Develop goals for improving treatment and care plans and communicate treatment and care plan to direct care staff.
- Evaluates residents’ cares
- Provides education and direction to staff as needed.
- Completes new resident and long-term care resident follow-ups to ensure they receive proper care.
- Leads care conferences to discuss resident conditions, treatments, and current plan of care, along with discuss new strategies that could be implemented.
- Coordinates the MDS process which includes scheduling and completing the MDS assessments and ensuring all sections are completed timely & thoroughly for all the residents. Will submit all MDS assessments to the CMS on a weekly basis.
- Ensure proper billing procedures are completed. Stays updated on current payment issues, interpretations and RAI/MDS updates; receives and shares information. Issues Notice of Medicare Non-Coverage (NOMNC) completion and Advance Beneficiary Notice (ABN) completion if applicable to resident stay.
- RN on-call rotation for care center and assisted living on weekends and holidays.
- Expectation to serve on QA/QI sub-committee or task forces.
- Active & integral participant of the MDH State Survey Process.
- Uses the EMR accurately & efficiently.
- Practices correct infection control procedures.
- Other duties as assigned

- Completes requirements for in-service and annual training, acceptable attendance, uniform dress codes including personal hygiene, and other work duties as assigned.
QUALIFICATIONS
- Current RN licensure in Minnesota.
- Medicare experience desired.
- CPR certification.
- Experience in LTC preferred.
- AAPACN certified in MDS or willingness to obtain within one year.
WORKING ENVIRONMENT/PHYSICAL DEMAND
- Prolonged periods standing and walking and frequently pulling, pushing, and bending.
- Must be able to lift and carry up to 35 pounds at times.
